New Student packets are available throughout the summer in the office

Parents must provide the following:
~ Copy of birth certificate
~ Copy of social security card
~ Copy of immunization records
~ Copy of guardian's driver's license
~ Proof of residency (two utility bills)
         OR
~ Residency affidavit
~ Custody papers (if child does not live with both biological parents)
~ Last report card
~ Drop/transfer slip if transferring within the parish
~ If applicable: Special Education 1508 evaluation and latest IEP 
                           504 evaluation and IAP
